The Paint Creek Dentistry Membership Plan is not dental insurance, a qualified health plan under the Affordable Care Act, pre-payment for future services or payment for access to discounted services. The membership plan is a direct payment arrangement for services, products and care provided. Membership plan benefits may not be combined with insurance or with others offers or discounts.

The membership plan includes a 30-day money back guarantee, and you can receive a full refund up to 30 days after purchase if no services or discounts have been used. Payments must be current to receive the services and discounts included in the plan, and any payments for treatment not included in the membership plan is due at the time of service.

Our membership plan does not cover procedures such as comprehensive orthodontic treatment, appliance therapy or adjunctive general services such as anesthesia.

The membership plan runs 12 months from the date of purchase and includes an auto renewal feature. You will receive an email notification 45 days prior to your renewal date, and you may opt out of auto renewal at any time.
